Grant Application Information
Considerations in applying for a direct care/services grant:
Grants are not provided for:
Housing
Incentives
Emergency funds
Drug treatment/management
Capital improvements
Grants may be made for:
Case management, if not funded elsewhere
Mother/child care for affected or infected
Condoms and other prevention barriers
Food programs
Legal support, if a part of connecting to available benefit programs
Salary augmentation, if evidence of sustainability is provided
Medication adherence management
The proposal must include:
The targeted population number served and demographics
The specific needs to be met (essential, basic needs/services not provided through Ryan White or other
public funds)
A narrative response to the questions in the application indicating how the program will be executed and
how outcomes will be measured
Downloading a Direct Care / Services Grant Application
We have simplified our grant application process by providing an
application in rich text format (RTF). You can download the application file, save it on your computer,
and complete the application form at your convenience, using any word processing application that supports
RTF (such as Microsoft® Word, WordPerfect®, and Adobe® FrameMaker®).
To ensure that your application will be considered in the granting process, please follow these guidelines for
completing the application form:
Use the application form's pre-set layout and styles.
Use font type ARIAL and font size 10 in the answer boxes.
Use the allotted length of the answer box spaces for your answers.
Stay within the allotted two pages of the application form.
Helpful hint: Use your mouse or arrow keys to navigate to the next field or text box instead of
using the tab key. If you accidentally create an extra row or box, use the Undo function to correct the error.

Prepare one copy of each of the following documents:
Evidence of your organization's tax-exempt status from the Internal Revenue Service under Section 501(c)(3)
Detailed line item budget for the program for which you are requesting funds (with a one- to two-sentence
justification for each item)
Your organization's overall operating budget
Your organization’s non-discrimination policy statement regarding your clients and services
